FAQ for App Users

A personal health record, otherwise known as a PHR, is a health record whereby the patient or user maintains their own health data and information.

The Lifecard app allows you to track your critical measurements and observations, safely store your medical history (including allergies, procedures and medicines) and also access health summaries, results, and letters submitted to your Lifecard by your healthcare team.

Your GP can send results directly to your Lifecard if they use an interoperable secure messaging vendor (ReferralNet or Argus), or you can simply add the measurements in or take a photo of your results and save to your Lifecard for storage.

Yes, you can easily export your Lifecard to a PDF.

Lifecard is available for download on the Apple App Store and Google Play. You can also use the web browser version on your desktop or laptop.

You can register with just your name and email, however we suggest inputting your Medicare number, date of birth and mobile number to get the most out of Lifecard.

Yes, you can share your record with other Lifecard users so that they have the information in case of an emergency. You can also export your record if you need to. However, we suggest that you put your emergency information into Apple Health or Google Fit so that it can be accessed without unlocking your phone.

FAQ for Patient Portal

Lifecard Patient Portal is an online portal for you to easily fill out forms prior to you visiting your hospital and clinic.

That’s okay. You can save your form and complete it at another time from any device.

The allergies and medications you record in your online form will be saved to your Lifecard Record for your convenience. The complete form will also be saved to your Lifecard for your reference in the future.

You can switch off the ‘sync’ to Lifecard feature. Your Lifecard login will remain so that you can access your past hospital and clinic forms, however the information you record on the patient portal will not be saved to your Lifecard account. Note, you will have to re-enter all of your medications and allergies next form you do for the hospital or clinic.
